The 2nd ESA Autumn Meeting Materials
Each delegate must present the barcoded registration confirmation letter at the ESA Secretariat Desk at the Park Inn Hotel. Each delegate will then receive the information folder together with the Meeting’s Badge.
The registration fee for participants includes:
- The conference programme
- Information related to the presentations
- Certificate of attendance
- Notepad and Pen
- Complimentary coffee break and lunch
Cancellation/ Refunds/ Name Changes
Registration cancellations and requests for refund of the registration fee for ESA Autumn Meeting must be received in writing at the ESA Secretariat (anny@euroanaesthesia.org) no later than 30 days (Tuesday, 11 October 2011) prior to the first day of the meeting. Beyond this period, refunds for cancellation are not applicable. Eligible refunds, net of a 50 euro administration fee, are transacted after the Meeting. Registration name changes are subject to a 50 euro administration fee.
Passport/ Visa/ Invitation Letter
Each delegate is responsible for having a valid passport for entry into Poland. Please check with the Polish Embassy in your country to determine if a VISA is required. Participants requiring a VISA for entry into Poland are advised to submit their application in their home country at least two months prior to the intended date of travel.
All registered delegates will receive an Invitation Letter to use if necessary to obtain a VISA. The Invitation Letter will be sent to you automatically together with your Registration Confirmation Letter.
The letter of invitation does not cover registration fees or any related expenses and is considered a formality to facilitate the process of obtaining a VISA for Poland for those delegates requiring this document. Cancellations due to VISA refusal/delay are non-refundable.
